Prevention: continue or cap the separation as required by the approved assembly, which means you can correct the load path or assembly while the work remains accessible.
Failure: placing an upper bearing wall over unsupported joists. Prevention: align support or obtain designed transfer framing, which means your approved repair is documented before the next trade arrives.
